this book and take no responsibility for the use of the products and procedures described. Treatments
and side effects described in this book may not be applicable to all people; likewise, some people may
require a dose or experience a side effect that is not described herein. Drugs and medical devices are
discussed that may have limited availability controlled by the Food and Drug Administration (FDA)
for use only in a research study or clinical trial. Research, clinical practice, and government regulations
often change the accepted standard in this field. When consideration is being given to use of any drug
in the clinical setting, the healthcare provider or reader is responsible for determining FDA status of
the drug, reading the package insert, and reviewing prescribing information for the most up-to-date
recommendations on dose, precautions, and contraindications, and determining the appropriate usage
for the product. This is especially important in the case of drugs that are new or seldom used.

Dedication
As always, I want to thank my publisher, Chris Davis, and friends
at Jones and Bartlett for the opportunity to collaborate with Michael
on this book. This book is dedicated to my spouse, Henry, and our
children, Isabelle, Harrison, and Sophia, who are my support and
inspiration for the work I do. I also want to dedicate it to the best
parents in the world, Millionita and Modesto Dizon, who gave me
every opportunity in the world to succeed from our little island in
the South Pacific. This is also for my sisters, Michelle, Maerica,
Precy, and Marie; their husbands, Mel, Ben, and Ed; and my niece
and nephew, Stella and Jude. Lastly, I want to dedicate this to the
people on my home island of Guam. Although far away, it will
always be home to me.
—Don S. Dizon, MD, FACP
To my healthcare team, I thank you for your support. For my par-
ents, the Franconis and Krychmans—a special thank you for the
unconditional encouragement during my career. To my co-author
Don Dizon, I am thankful for our friendship and am honored to
have you as an esteemed colleague in oncology and sexual medi-
cine. To my good friend and colleague, Susan, who has always
encouraged me— your friendship has been a source of inspiration.
For Bryanna, I hope all this talk about HPV has not completely
scared you. Thank you to Chris Davis and the Jones and Bartlett
team for their hard work and dedication.
And last but not least, for all the men and women, young and
old, married, single, divorced, heterosexual and homosexual, who
are desperate for education about HPV, its effects, and its treat-
ment. I do hope this book can help ease the suffering and distress.
—Michael L. Krychman, MD
